Found this gem in my rear run flat. Carefully remove with some pliers? Just a flesh wound? Business as usual after I remove or take it in to get inspected?

I think you lucked out. Perhaps some plug repair cement in the hole just because you can and call it a day.

It is run flat? I was told I’d need to get new one if it got punctured but they were able to fix it with a rubber plug thingy and I drove without any issues since then.

I removed it, and from what I can tell, it was just a surface wound through the tread but easily could have done worse. Seems to be holding pressure, but I'm going to have someone look at it regardless.
