Lug nut key

The strangest thing happened to me today. The lug nut key that works on all four lug nuts does not fit on the rear left wheel. I do not see any signs of tampering. How can this be possible? Has anyone encountered this issue? I have the vehicle booked to be taken looked at. I can't seem to picture what may have caused this problem.
